If you are covered by a Health Maintenance Organization (HMO), a Preferred Providers Organization (PPO) or some other managed care program, please let us know when you make your appointment. Most insurers require special advance authorization before treatment can begin. You will need to have a valid referral/authorization on file (if required by your insurance).

Per your contract with your insurance company, your co-pay and/or deductible are your responsibility and will be collected by Schulze Orthopedics at the time of service. If you are unsure of your co-pay and/or deductible responsibility, contact your insurance provider at the (800) number provided on your insurance card. If your insurance denies coverage you are responsible for services rendered.
Please bring your insurance card and valid drivers license or ID to your scheduled appointment.
